Certified Birth Certificate.
Copy of Identity Card, both parents or guardian.
2 2×2 photos.
Copy of Vaccination Record/Vaccination Certificate.
Ophthalmologic certificate.
Hearing evaluation certificate (from 2-6 years old).
Letter of balance from previous institution/ if applicable.
School certificate from previous institution.
Certificate of completion of Primary Level (6th grade) if applicable.
Grade transcript (if high school student).
Registration authorization from the Department of Validation of the Ministry of Education (if coming from abroad).
